Individuals from 25 to 69 years old with a cervix can choose to order a kit to self-screen for the human papillomavirus (HPV), the leading cause of cervical cancer. The new cervical screening test is similar to a Pap smear test.

We offer a variety of preventative care services, chronic disease follow-up, wellness exams, third party medicals, drivers medicals, integrated health nurse, and more.

Please book an appointment with your practitioner in advance to have any forms completed. Please ensure you have completed your section and signed the authorization. Please note there is a minimum fee of $20 for forms and $55 for disability tax credit form completion.
